 Our office is conveniently located on Siskiyou Blvd. in the new Siskiyou Medical Building near Rogue Valley Hospital.

Appointments

Insurance
Our office accepts most insurance health plans. Please call our office to verify acceptance of your insurance carrier.

Payment Options
Payment is expected at the time of service for all non-covered services and over-the-counter products. We expect all co-pays to be paid at the time of service as well. We will bill your insurance company for you. We accept VISA and Master Card as well as checks and cash. Any other payment arrangements must be made through our office manager.
